🎯 Task

CANDIDATE TASK

A 24-year-old woman at 18 weeks of pregnancy attends routine antenatal care and is at risk of developing iron-deficiency anemia. Write an appropriate preventive prescription and provide the important instructions for correct use of the prescribed iron preparation.

Prescription: Ferrous fumarate 180 mg tablet, providing approximately 60 mg elemental iron, orally once daily.

Directions: Take one tablet daily throughout pregnancy; preferably away from meals if tolerated, but it may be taken with food if gastrointestinal upset occurs.

Counselling: Avoid taking it together with tea, coffee, calcium or antacids. Explain that nausea, constipation and dark stools may occur.

Safety: Write the prescription clearly using the generic name, specify dose, route, frequency and quantity, then date and sign it.

🔎 Stepwise Procedure / Approach

  1. Review the scenario. Confirm the indication for preventive oral iron and check for relevant allergy, contraindication or intolerance.
  2. Enter patient details and date. Record sufficient patient identification before writing the medication order.
  3. Write the Rx clearly. Use the generic drug name and specify the dosage form and strength.
  4. State the exact regimen. Write the dose, oral route and frequency without ambiguous abbreviations.
  5. Specify quantity or duration. Ensure the amount supplied is consistent with the intended treatment period and follow-up.
  6. Give clear patient instructions. Explain daily administration, measures that improve absorption and what to do if gastrointestinal intolerance occurs.
  7. Counsel about common effects. Mention nausea, abdominal discomfort, constipation and harmless darkening of stools.
  8. Check and authenticate. Re-read the prescription for errors, then add the prescriber’s required identification and signature.
