TOPIC 3. Obstructive Jaundice, Pancreatic Malignancy & Jaundice in Special Populations
Connect key mechanisms, clinical clues, investigation findings and management decisions for rapid revision.
1. THE TOPIC IN ONE CONNECTED FLOW
Jaundice starts with altered bilirubin handling, but the clinical significance depends on where the problem occurs and who the patient is. In adults, impaired bile drainage prompts investigation for obstruction and pancreatic malignancy. In infants and children, the bilirubin fraction separates important causes; in pregnancy, associated maternal and fetal risks guide urgency and treatment.
2. KEY CLINICAL CONNECTIONS
Adult biliary obstruction and pancreatic head cancer
Conjugated bilirubin ↑ + ALP ↑ + ductal dilatation → mechanical obstruction → ultrasound first, then localization and treatment.
Painless progressive jaundice + weight loss + pancreatic head mass → CT assessment of vascular involvement / spread → resection if suitable, or symptom-relieving biliary drainage and patient-centred counselling.
Newborn jaundice and Gilbert syndrome
Newborn age in hours + measured bilirubin + gestational/risk context → treatment threshold → phototherapy when indicated; pale stools + direct bilirubin ↑ → prompt cholestasis assessment.
Well adolescent + intermittent unconjugated bilirubin ↑ + normal liver tests / no hemolysis → Gilbert syndrome → reassurance and simple trigger advice.
Pregnancy-related hepatobiliary disease
Third-trimester itching without a primary rash → serum bile acids ↑ → obstetric cholestasis → symptom control, fetal risk assessment and delivery planning.
Late-pregnancy vomiting + jaundice + hypoglycemia / coagulopathy → suspected acute fatty liver → urgent maternal stabilization and expedited delivery planning.
3. AIM HIGH-YIELD INTEGRATION REVIEW
- ⭐ Conjugated jaundice + raised ALP + dilated ducts → obstructive pattern → locate cause before choosing intervention.
- Fever + pain + jaundice, especially with hypotension → acute cholangitis → antibiotics, resuscitation and urgent drainage.
- ⭐ Painless progressive jaundice + weight loss → suspect pancreatic head cancer → pancreatic-protocol CT guides staging and resectability.
- In unresectable pancreatic cancer → biliary stenting may relieve jaundice and itching, but does not remove the malignancy → explain palliative goals sensitively.
- ⭐ Jaundice in the first 24 hours or high-risk neonatal bilirubin → prompt measured assessment; unconjugated bilirubin neurotoxicity → age/risk-based treatment.
- Persistent infant jaundice + pale stools + conjugated bilirubin ↑ → possible biliary atresia → prompt referral, not routine phototherapy.
- Intermittent mild unconjugated bilirubin ↑ with normal enzymes and no hemolysis → Gilbert syndrome → benign reassurance, no routine treatment.
- ⭐ Third-trimester itching + bile acids ↑ → obstetric cholestasis; vomiting + hypoglycemia + coagulopathy → possible acute fatty liver → emergency maternal care.
