Candidate Task

Examine the supplied model, image or specimen demonstrating a postmortem change. Identify the change, describe its characteristic features, differentiate it from a commonly confused finding where appropriate, and state its basic medico-legal significance.

Identification: I would identify the demonstrated change from its dominant characteristic appearance.

Key recognition: Dependent bluish-purple discoloration suggests postmortem lividity; generalized muscle stiffening suggests rigor mortis; green discoloration, marbling and bloating suggest putrefaction; a waxy greasy appearance suggests adipocere; and dry, shrunken leathery tissue suggests mummification.

Differentiation: Where relevant, I would distinguish the finding from antemortem injury or another postmortem phenomenon using its distribution and characteristic morphology.

Significance: Postmortem changes help in estimating the postmortem interval; lividity may also indicate body position or subsequent movement. No single postmortem change gives an exact time of death.

🔎 Stepwise Procedure / Approach

  1. Observe systematically. Assess colour, distribution, surface appearance, posture, tissue consistency and any obvious decomposition.
  2. Identify the dominant clue. Look specifically for dependent discoloration, muscular stiffness, green discoloration, marbling, bloating, waxy change or dry shrinkage.
  3. Consider the type of change. Decide whether the finding represents an early change such as lividity or rigor, or a later change such as putrefaction, adipocere or mummification.
  4. Check the distribution. For lividity, examine dependent areas and look for pressure-related areas of pallor. For decomposition, assess the pattern and extent of tissue change.
  5. Exclude a common mimic. If discoloration is shown, distinguish postmortem lividity from bruising; if stiffness is demonstrated, distinguish rigor from other causes of postmortem stiffness where appropriate.
  6. State the identification clearly. Name the postmortem change and describe two or three strongest supporting features.
  7. State medico-legal significance. Relate the finding to estimation of the postmortem interval and, where applicable, body position or movement after death.

Q1. What is postmortem lividity?

Postmortem lividity is a bluish-purple discoloration of dependent parts of the body caused by gravitational settling of blood in vessels after circulation has ceased.

Q2. How can postmortem lividity be differentiated from a bruise?

Lividity usually occurs in dependent areas and blood remains mainly within vessels. In a bruise, blood is extravasated and infiltrates the tissues at the injured site.

Q3. What is the mechanism of rigor mortis?

After death, depletion of ATP prevents separation of actin and myosin within muscle fibres, producing postmortem muscular rigidity.

Q4. What is the usual progression of rigor mortis?
