Topic 10 — Bladder and Prostate Malignancies

4th Year MBBS • Renal Module • High-yield memory reinforcement and last-minute revision

1. High-Yield Flashcards

Tap each question to reveal the answer.

What is the most important tumor group arising in the urinary bladder?
Urothelial tumors arising from the transitional epithelium.
Which two broad growth patterns are important in urothelial carcinoma?
Papillary tumors and flat carcinoma in situ.
What is the major preventable risk factor for urothelial carcinoma?
Cigarette smoking.
What microscopic structure characterizes a papillary urothelial tumor?
Fibrovascular cores covered by proliferating urothelial cells.
Why is muscularis propria invasion important in bladder carcinoma?
It defines muscle-invasive disease and markedly changes prognosis and management.
Which procedure both removes visible bladder tumor and provides tissue for staging?
Transurethral resection of bladder tumor (TURBT).
Which intravesical treatment is important in high-risk non-muscle-invasive bladder cancer and CIS?
Intravesical BCG.
Where does prostate adenocarcinoma commonly arise?
The peripheral zone of the prostate.
What is an important precursor lesion associated with prostate adenocarcinoma?
High-grade prostatic intraepithelial neoplasia (high-grade PIN).
What type of bone metastasis is classically associated with prostate carcinoma?
Osteoblastic or sclerotic metastasis, especially in the axial skeleton.
Why can PSA not diagnose prostate carcinoma by itself?
PSA is prostate-specific but not cancer-specific and may rise in benign prostatic conditions.
How does continuous goserelin treatment reduce testosterone?
Continuous GnRH stimulation causes pituitary receptor desensitization and down-regulation, reducing LH and testosterone.
What important early effect may occur after starting a GnRH agonist?
A temporary testosterone rise causing tumor flare.
What is the main mechanism of flutamide?
Competitive blockade of androgen receptors.
How do docetaxel and cabazitaxel inhibit prostate cancer cell proliferation?
They stabilize microtubules, disrupt normal mitosis and inhibit malignant-cell proliferation.

2. Mnemonics

Mnemonic Title: Bladder Cancer Exposure Risks
SMAC
Meaning: Smoking • aromatic aMine occupational exposure • cyclophosphAmide • Chronic irritation. Chronic irritation is particularly linked to squamous differentiation.
Mnemonic Title: Bladder T-Stage Depth Ladder
SLaMPA
Meaning: Surface papillary lesion → Lamina propria → Muscle → Perivesical tissue → Adjacent structures.
Mnemonic Title: Taxane Action and Toxicity
TAN
Meaning: Tubules stabilized → mitotic Arrest → Neuropathy and neutropenia/myelosuppression are important toxicities.

3. Memory Tables

Non-Muscle-Invasive vs Muscle-Invasive Bladder Cancer

Feature Non-Muscle-Invasive Muscle-Invasive
Depth Ta, Tis or T1 T2 or deeper
Main local treatment TURBT Definitive local treatment such as radical cystectomy
Additional therapy Intravesical therapy in selected disease Systemic chemotherapy may be incorporated
Key concern Recurrence and progression Regional and distant spread

GnRH Agonist vs GnRH Antagonist

Feature GnRH Agonist GnRH Antagonist
Example / principle Goserelin Direct GnRH-receptor blockade
Initial effect Temporary LH/testosterone rise Rapid LH/testosterone fall
Tumor flare May occur No initial flare
Final therapeutic effect Marked androgen suppression Marked androgen suppression

4. Rapid Revision Points — Last-Minute Revision

Must Remember:
  • Most bladder cancers are urothelial in origin.
  • Carcinoma in situ is a flat, high-grade urothelial lesion with important progression potential.
  • Ta is non-invasive papillary disease; T1 reaches lamina propria; T2 reaches muscularis propria.
  • Painless hematuria is a major clinical clue to bladder malignancy.
  • Urine cytology is more useful for high-grade urothelial lesions and CIS than for low-grade papillary tumors.
  • Prostate carcinoma commonly begins in the peripheral zone, unlike the periurethral distribution of BPH.
  • PSA supports assessment and follow-up but is not diagnostic of cancer by itself.
  • Prostate carcinoma classically produces osteoblastic skeletal metastases.
  • Goserelin causes an initial androgen flare before sustained testosterone suppression.
  • Flutamide blocks androgen receptors and can cause important hepatotoxicity.
  • Cyproterone acetate blocks androgen receptors and also suppresses gonadotropin secretion.
  • Taxanes used in advanced prostate carcinoma may cause myelosuppression and peripheral neuropathy.
KMU Exam Trap: TURP may improve obstruction but is not equivalent to radical prostatectomy and does not provide definitive oncological treatment for localized prostate carcinoma.

5. Clinical Memory Hooks

Painless visible hematuria in an older smoker
→
Suspect urothelial malignancy and evaluate the bladder.
Velvety erythematous bladder mucosa with irritative symptoms
→
Think carcinoma in situ; urine cytology may be informative.
Hard irregular prostate with suspicious imaging
→
Tissue biopsy confirms carcinoma and provides histological grade.
Prostate cancer with new back pain and leg weakness
→
Consider metastatic spinal involvement and arrange urgent specialist assessment.
Worsening bone pain soon after starting goserelin
→
Initial testosterone surge causing tumor flare.

6. Starred High-Yield Exam Points

  • ⭐ Muscularis propria invasion converts bladder carcinoma into muscle-invasive disease and changes management substantially.
  • ⭐ TURBT provides both tissue diagnosis and assessment of bladder tumor invasion.
  • ⭐ Prostate adenocarcinoma commonly arises in the peripheral zone and may remain asymptomatic early.
  • ⭐ Osteoblastic axial skeletal metastases are a classic finding in metastatic prostate carcinoma.
  • ⭐ Continuous goserelin eventually suppresses LH and testosterone through GnRH-receptor desensitization/down-regulation.
  • ⭐ Flutamide is an androgen-receptor antagonist with clinically important hepatotoxicity.
  • ⭐ Docetaxel and cabazitaxel stabilize microtubules, impair mitosis and are used in selected advanced prostate carcinoma.
