- Acute angle closure causes pain, red eye, visual reduction, corneal edema, shallow chamber and a mid-dilated poorly reactive pupil.
- Gonioscopy identifies the angle mechanism; tonometry measures the pressure rise.
- Acute pupillary block requires immediate IOP lowering followed by definitive peripheral iridotomy.
